Palliative Care Nurse Practitioner Georgetown, SC

The Palliative Care Nurse Practitioner (PCNP) works in collaboration with Tidelands Community Palliative Care staff to provide comprehensive, quality palliative care services to patients on their program. The PCNP will be responsible for providing patient visits for palliative care patients, both new and established and coordinating their plan of care with the palliative care team and other providers involved with their care. Patient visits will include visits to patient’s homes and facilities. In addition, The PCNP will engage in independent decision-making about a patient’s healthcare concerns and provide palliative care services as needed. Tidelands Community Palliative Care medical directors will be available for collaboration.
